Art therapy is a professional mental-health treatment that uses drawing, painting, collage, clay, and other creative processes within psychotherapy. It is not the same as coloring alone, taking an art class, or buying an art-therapy workbook. Research suggests it may help some people with particular goals, but the evidence is mixed and does not support claims that art therapy cures conditions, diagnoses people from pictures, or works for everyone.
What is art therapy?
Art therapy is a professional mental-health service that combines active art-making, psychological theory, and a therapeutic relationship. A credentialed art therapist may invite a client to draw, paint, work with clay, make a collage, or keep a visual journal while helping the client explore emotions, experiences, relationships, behavior, and personal meaning.
It is not the same as taking an art class, coloring alone, or buying an art-therapy workbook. Those activities can be enjoyable forms of recreation, mindfulness, or self-care, but they do not create a therapist–client relationship and are not a substitute for clinical treatment.

You do not need to be artistic or produce attractive work. In art therapy, the therapeutic process may matter more than the finished image: the choices you make, your sensory experience, your emotional response, the associations an image evokes, and what you discover while discussing it with the therapist.

How art therapy works
The American Art Therapy Association describes art therapy as a mental-health profession facilitated by a professional art therapist. Art therapists use the creative process within psychotherapy to support goals that may be personal, relational, emotional, social, cognitive, sensorimotor, community-based, or related to coping.
A session might include any combination of the following:
- Checking in and setting a focus: You and the therapist may discuss how you have been feeling, what has happened since the last session, and what you want to work on.
- Making something: The therapist may offer a prompt, suggest a material, ask you to create a visual metaphor, or leave the activity open-ended. You might draw an image, paint, arrange a collage, shape clay, or combine writing with visual work.
- Reflecting: The therapist may ask what you notice about the process, what the image means to you, or how it connects with your current experience.
- Connecting the work to treatment: The discussion can relate the activity to coping skills, communication, grief, identity, trauma recovery, stress, relationships, or another agreed clinical goal.
- Closing safely: The therapist helps you transition out of the activity, identify anything that needs further attention, and plan what happens next.
There is no universal art-therapy exercise or mandatory sequence. The intervention should be adapted to the client’s age, goals, culture, disability, trauma history, sensory needs, developmental level, and safety. A therapist should explain the activity, respect your boundaries, and adjust or stop it if the process becomes overwhelming.

A responsible art therapist does not treat a particular color, shape, subject, or drawing style as a fixed diagnostic code. A picture cannot automatically reveal a hidden disorder or prove what happened to someone. Meaning is explored collaboratively and in context. The client’s own associations, words, history, behavior, and therapeutic goals matter more than rigid symbolism.
The therapist may be interested in how you approach an activity, what choices feel difficult or comfortable, how you respond to frustration, or what you notice while making the work. The finished artwork is only one part of that process.
Common art-therapy techniques and materials
Art therapists may use many of the same materials found in a studio, although the purpose is therapeutic rather than artistic performance. The technique is selected to fit the person and the treatment plan, not because one medium has a guaranteed psychological effect.
| Technique or medium | What it may offer in therapy | Important qualification |
|---|---|---|
| Drawing and mark-making | Free drawing, guided prompts, self-portraits, visual journaling, and image-based reflection can give shape to feelings or experiences that are difficult to explain verbally. | The image is not a diagnostic test. The client and therapist explore its meaning together. |
| Painting | Watercolor, acrylic, tempera, and other paints can support experimentation, color exploration, sensory engagement, and nonverbal expression. | Paint can be messy, stimulating, or inaccessible for some people. Materials should be adapted to sensory, physical, and safety needs. |
| Collage | Arranging pictures, words, textures, and found materials provides a structured starting point and may support identity, narrative, or meaning-making work. | Choosing from existing images can feel easier than starting with a blank page, but some images may also be emotionally activating. |
| Clay and three-dimensional work | Modeling, building, and tactile manipulation add sensory and motor elements and can make it possible to work without relying primarily on speech. | Clay may not suit people with certain sensory sensitivities, physical limitations, allergies, or environmental restrictions. |
| Coloring and patterned work | Coloring can be used as one contained activity for attention, experimentation, or creative engagement within a broader therapy session. | An adult coloring book used independently is recreation or self-care, not equivalent to treatment with an art therapist. |
| Visual journaling and mixed media | Combining writing with drawing, collage, painting, or other media can help represent an experience from several angles. | Journaling can bring up difficult material. A therapist can help establish limits and coping steps when needed. |
Who may use art therapy?
Art therapists work with children, adolescents, adults, families, and groups. They may practice in hospitals, outpatient clinics, schools, community programs, wellness settings, and private practices. Art therapy may be considered for concerns such as stress, grief, emotional regulation, identity, relationship difficulties, adjustment to illness, or mental-health conditions. The appropriate treatment depends on the individual assessment, the therapist’s training, and the client’s preferences.

Art therapy can be particularly appealing to people who find it hard to begin with direct verbal discussion, although speaking and reflecting are often still part of the work. It should not be presented as a way to avoid all conversation or assessment. A qualified clinician can decide when art-making is appropriate and how it fits with other therapeutic methods.
How effective is art therapy?
The evidence is promising in some areas but not definitive. Studies have reported improvements in selected symptoms or quality-of-life outcomes, yet results vary substantially by population, intervention, duration, therapist training, comparison treatment, and outcome measure. Many studies are small or methodologically weak. The most accurate general conclusion is that art therapy may help some people with some goals; research does not show that it works for everyone or that one technique is best.
What systematic reviews have found
| Population or intervention | Findings | How confidently to interpret them |
|---|---|---|
| Active visual art therapy overall | A large systematic review and meta-analysis examined 50 studies involving 2,766 people and 217 outcomes. A minority of measured outcomes improved compared with controls, while most did not. | The studies were highly varied and generally low quality. The review supports cautious, outcome-specific claims rather than a broad promise that art therapy is effective in general. |
| Group art therapy for non-psychotic mental-health disorders | A review of 11 randomized controlled trials involving 533 patients found significant positive changes in seven studies compared with controls. | The trials were generally poor quality and at high risk of bias. More research is needed, especially comparisons with established verbal therapies. |
| Anxiety in adults | A review of randomized and non-randomized controlled trials found a developing evidence base. | Interventions and outcome measures varied considerably, so the findings do not establish a single reliable protocol or guaranteed anxiety reduction. |
| Depressive symptoms in adults | A 2024 systematic review and meta-analysis of visual art therapy found that effects differed according to the population and intervention duration. | Results from one group, duration, or visual-art program should not be generalized to every adult with depression. |
| Cancer care | A 2020 review and meta-analysis of 12 studies involving 587 patients reported improvements in anxiety, depression, fatigue, and quality of life. | Art therapy was evaluated as complementary care. It should not replace oncology treatment, medical advice, or symptom management. |
| Children and adolescents with autism spectrum disorder | A 2025 review of 12 randomized controlled trials reported possible improvements in autism-related symptoms, stress, social communication, motor skills, language, and neurodevelopment. | Most studies had small samples, short durations, high risk of bias, or low methodological quality. The results are encouraging but preliminary. |
| Children with medical conditions | A review of 12 studies involving 404 pediatric participants found improvement in at least one outcome in 10 studies. | The overall trend was inconclusive and the evidence quality was weak. |
| Mandala-based interventions in adults aged 55 and older | A 2026 meta-analysis of nine randomized trials involving 648 participants reported lower anxiety and depressive-symptom scores. | This was a narrow intervention with substantial heterogeneity and very-low-certainty evidence. It is not proof that mandala coloring or art therapy generally treats anxiety or depression. |
These findings also illustrate why the phrase “art therapy works” is too broad. A study may measure a short-term change in anxiety during cancer treatment, for example, while a different study examines social communication in autistic children. Those results cannot simply be combined into a promise about every person or every form of art-making.
What art therapy should not claim to do
Be cautious if a provider, course, book, or product promises any of the following:
Free tools Windows power users keep installed
One-click scans. No signup required.
- That a drawing can diagnose a mental illness or reveal a hidden trauma without discussion and clinical assessment.
- That a specific color, symbol, or image has one universal psychological meaning.
- That art therapy will cure trauma, depression, anxiety, or another condition.
- That art-making can replace medication, psychotherapy, medical care, or crisis services for everyone.
- That a coloring book or supply kit is itself professional art therapy.
Art therapy may complement standard care, or it may be one approach considered when conventional verbal treatment is difficult or less acceptable. Whether it is appropriate should be decided with a qualified professional and, where relevant, the person’s medical or mental-health care team.
Art therapy versus creative self-care
You can receive real benefits from making art without calling the activity art therapy. Drawing, painting, collage, clay, coloring, and visual journaling may offer recreation, a pause from screens, focused attention, or a way to express yourself privately. They can be worthwhile even when there is no clinical goal.
If you want to experiment at home, an optional kit could include colored pencils for art making, a sketchbook or mixed-media journal, markers, watercolor or other paints, paper, collage materials, or air-dry clay. An adult coloring book for creative self-care may also be a simple, low-cost activity. These materials are tools for creative practice—not treatment—and should not be marketed as a replacement for a clinician.

Credentials and legal requirements depend on where the practitioner works. The information below applies primarily to the United States.
U.S. credentials
The Art Therapy Credentials Board identifies several practitioner levels relevant to consumers:
- ATR-P: Provisional Registered Art Therapist.
- ATR: Registered Art Therapist.
- ATR-BC: Board Certified Registered Art Therapist. This credential requires the ATR plus successful completion of the board-certification examination.
- ATCS: An additional credential related to supervision.
The American Art Therapy Association states that a master’s degree is the entry-level educational requirement for art-therapy work. Graduates may then pursue national credentialing and state licensure, depending on where they practice. National credentialing and state licensure are separate: an ATR-BC does not automatically establish permission to practice independently in every state.
Credential maintenance also has different schedules. The underlying ATR requires annual renewal, while board certification is maintained through a five-year recertification cycle. Confirm current requirements directly with the relevant credentialing organization.
Check the law where you live
U.S. states differ. Some have standalone art-therapy licenses; others regulate art therapy under a related mental-health license or recognize art therapists through title protection or state employment rules. Before booking, check your state licensing board and ask the practitioner what license or authorization permits them to provide the service in your location. If the session is online, verify the rules for the state or jurisdiction where you are physically located during treatment.
The American Art Therapy Association maintains an Art Therapist Locator, and the Art Therapy Credentials Board provides credential-verification resources. These tools can help you find an art therapist or verify an art therapist credential, but directory inclusion is not an endorsement and does not guarantee clinical fit, availability, or quality.

Questions to ask before your first appointment
- What graduate training, credentials, and licenses do you hold?
- Are you licensed or otherwise authorized to practice in my state or jurisdiction?
- What populations and concerns do you regularly treat?
- How do you use art-making alongside assessment, conversation, and other therapeutic methods?
- What happens if an activity feels overwhelming, activating, or unsafe?
- How do you adapt materials for disability, sensory sensitivity, culture, language, or physical limitations?
- Who can see my artwork, how is it stored, and what happens to it when treatment ends?
- How do confidentiality, records, telehealth, payment, and insurance work?
- What is the plan if you believe I need a different type of care or a higher level of support?
A good first conversation should leave room for consent and questions. You should not be pressured to show work, disclose more than you are ready to discuss, or continue an activity that feels unsafe.
When art therapy is not enough
Art-making alone is not an emergency intervention. If you have suicidal thoughts, an immediate safety concern, psychosis, severe substance-related risk, or urgent medical symptoms, seek timely help through local emergency services, an emergency department, a crisis service, or an appropriate medical or mental-health professional. In the United States, call or text 988 for the Suicide & Crisis Lifeline; elsewhere, use the emergency or crisis service available in your country.
Recommended Free Tools
For non-urgent concerns, art therapy can be discussed with a primary-care clinician, licensed mental-health professional, or qualified art therapist. It may be used alongside other treatment, but the treatment plan should match the seriousness and nature of the problem.
Frequently asked questions
Do I need to be good at art to try art therapy?
No. Artistic skill is not a prerequisite. The focus is on the experience of making, reflection, communication, and therapeutic goals—not on producing technically impressive work.
Is coloring in an adult coloring book art therapy?
Not by itself. Coloring can be a relaxing or mindful self-care activity and may also be one activity used during a session, but clinical art therapy involves a qualified art therapist and a therapeutic relationship.
Can an art therapist tell what is wrong from my drawing?
No responsible clinician should diagnose you from a single image or assign universal meanings to colors and symbols. Interpretation should be collaborative, contextual, and informed by assessment and conversation.

Can art therapy replace medication or regular psychotherapy?
Art therapy may complement standard care, but it is not a universal replacement for medication, psychotherapy, oncology care, or emergency services.
How can I check whether an art therapist is qualified?
Ask about graduate training, ATR or ATR-BC credentials, state licensure or authorization, experience with your concerns, confidentiality, and fees. Verify the information with the appropriate state licensing board and credentialing organization.
The Bottom Line
Art therapy is clinician-led mental-health treatment that uses art-making as part of psychotherapy—not a coloring product, art class, or diagnostic shortcut. Research suggests possible benefits for particular people and outcomes, but the evidence is mixed and often low-certainty. If you pursue it, verify the practitioner’s training and local authorization, agree on boundaries, and treat art-making at home as self-care rather than a substitute for professional or emergency care.
