Good aged-care architecture makes infection prevention easier to carry out. The strongest designs provide controllable fresh air, appropriate filtration, small household groupings, separable zones, private rooms and bathrooms where feasible, and handwashing, storage and personal protective equipment (PPE) support at the point of care. These features create workable conditions for prevention and outbreak response; they do not guarantee that infections will be stopped. Design must operate with risk assessment, cleaning, staffing, clinical practice and an outbreak plan.

What infection-control design needs to achieve

An aged-care building has to function as a home, a workplace and, during an outbreak, a containment system. Layout decisions influence how far residents, staff, visitors, air and equipment move. They also determine whether staff can isolate a resident, keep a household together, clean high-touch surfaces and obtain PPE without repeatedly crossing other groups.

The Australian Government Department of Health and Aged Care’s National Aged Care Design Principles and Guidelines (2024) is the most directly applicable source for these decisions. It presents environmental design as an enabling layer of infection prevention, not as a replacement for operational infection-prevention and control (IPC) procedures or local building requirements. Requirements differ by country, state, facility type, climate and existing mechanical systems, so a project team should verify the current local code and regulator guidance before turning a recommendation into a specification.

Start with air: room-level and building-level controls

Make fresh air usable

The 2024 Australian guide recommends routine air-quality checks and practical access to outdoor air. Windows, doors and fans can create cross-ventilation when outdoor conditions, resident safety and the building’s systems permit. Residents should be able to understand and control their environment, including whether and how windows can be opened. Controls that are too complicated, inaccessible or routinely disabled do not provide a reliable benefit.

Opening a window is not automatically appropriate. Outdoor pollution, smoke, heat, cold, noise, security, falls risk and the existing heating, ventilation and air-conditioning (HVAC) design may make natural ventilation unsafe or ineffective. If spot checks show unacceptable air quality, the guide advises consulting an engineer or another experienced ventilation professional before changing the system.

Keep bedroom and bathroom exhaust from shared areas

Where the system allows, air from bedrooms and bathrooms should be directed outdoors rather than recirculated through shared spaces. This principle limits the movement of contaminants from a potentially affected room into a household lounge, corridor or dining area. The correct solution depends on the existing ductwork, pressure relationships, weather and code requirements; it is not a direction to disconnect or alter equipment without professional assessment.

Use filtration as a supplement

The guide supports air-cleaning devices fitted with an appropriate HEPA filter. A portable purifier can be useful in a room or zone where outdoor-air delivery is limited, but it does not replace engineered ventilation, maintenance of the HVAC system or an outbreak plan. Select and position equipment through a competent assessment of room volume, airflow, noise, electrical safety, filter replacement and resident access. The official material does not validate a particular brand, model, room-size claim, price or certification.

Interpret CO₂ readings carefully

CO₂ monitors can be used for spot-checking CO₂ as a proxy for general air quality. The same 2024 guide gives two different reference values: its main clean-air checklist says to keep CO₂ below 1000 ppm, while its infection-control appendix says below 880 ppm. These figures should not be silently treated as interchangeable or as a universal infection threshold. They are government design recommendations in one document, and the guide describes CO₂ as a general-air-quality proxy rather than a direct measurement of infection risk.

Clean-air checklist CO₂ below 1000 ppm Use as the value stated in that section of the Australian 2024 design guide; investigate readings that exceed it.
Infection-control appendix CO₂ below 880 ppm Use as the separate value printed in the appendix, not as a replacement for the 1000 ppm statement.
Air-quality response Professional assessment when results are unacceptable Ask an experienced ventilation professional or engineer to assess the building and recommend corrective work.

Use smaller households to limit movement

The Australian guide recommends households of 15 people or fewer, with dedicated staff, defined boundaries and identifiable household entries. It also recommends smaller-group dining and at least one living space that can be segregated. These are design recommendations, not a universal statutory maximum, but they give a project team a concrete way to reduce unnecessary mixing.

A household entry should be recognisable and closable. Where appropriate, a vestibule can act as a buffer between the household and a circulation route. The boundary should be legible to residents, visitors, cleaners and agency staff; a boundary that exists only on a plan but is crossed constantly in practice will not provide meaningful separation.

Smaller groups can reduce staff and resident movement and make cohorting more manageable. Cohorting means grouping people according to exposure or infection status under the facility’s clinical plan. The layout should support several possible zones rather than assume one fixed outbreak pattern. As the design guide states: “Residential aged care homes need to develop a plan for managing an infectious disease outbreak, including options for zoning.”

Plan dining, living and circulation for separation

Replace one large communal room with flexible choices

Large, high-traffic dining or living rooms make it harder to separate groups quickly. Provide smaller activity and dining settings, and design at least one living area so it can be closed or otherwise segregated. Flexible furniture, robust partitions and doors that staff can operate safely can help, provided they do not create trip hazards, obstruct evacuation or compromise supervision.

Reduce unnecessary corridor traffic

Residents should not have to pass through another household to reach a bedroom, bathroom or outdoor area. Place frequently used services close to the residents who need them, and avoid making corridors storage areas. Clutter adds touchpoints, obstructs cleaning and can force people into close contact while equipment is moved.

Make entries and routes understandable

Clear wayfinding supports both ordinary daily life and rapid changes during an outbreak. Staff should be able to identify household boundaries, clean and dirty routes, PPE points and alternative paths without relying on improvised signs. Any temporary zoning arrangement should be compatible with fire safety, accessibility, emergency egress and resident dignity.

Bedrooms and bathrooms: privacy with response options

Prioritise single rooms where feasible

The 2024 guide recommends lockable single rooms with external windows and ensuite shower rooms where possible. A private room and bathroom can support privacy, reduce sharing and provide an option for isolating a person with suspected or confirmed infection, subject to clinical direction and applicable standards. This is a design capability, not a substitute for isolation protocols, observation, treatment or public-health instructions.

Design shared rooms for dignity and cleaning

Where shared rooms remain necessary, maintain privacy between bed areas with suitable screening and enough clearance for care and cleaning. Keep high-touch elements accessible for disinfection without requiring staff to move personal belongings. Room layouts should allow safe transfer of residents and equipment while limiting contact with the other bed space.

Connect rooms to safe outdoor access

External windows can provide daylight, views and potential fresh-air access. Outdoor areas also support activity and can be useful during periods when residents are isolated. Paths to those areas should avoid forcing an isolated resident through another household or a crowded communal room.

Specify finishes and storage that make good practice possible

Make high-touch surfaces cleanable

Door hardware, handrails, light switches, lift controls, tap handles and other frequently touched elements should be easy to reach, inspect and clean. Select finishes for durability and compatibility with the facility’s approved cleaning products. Avoid unnecessary ledges, joints and decorative details that trap dust or moisture. The guide does not establish that a particular coating or “antimicrobial” product prevents infection, so do not use a product claim as a substitute for cleaning.

Provide handwashing where care occurs

Handwashing support should be close to resident-care activities, with fixtures and consumables that staff can use without leaving a resident unattended. Placement should account for splash control, accessibility, hot-water safety, replenishment and visibility. Alcohol-based hand rub stations can supplement, but not replace, handwashing where required by the IPC procedure.

Put supplies and PPE at the point of use

Provide sufficient storage near the activity it serves, including discreet storage for clinical supplies. The guide recommends discreet PPE donning and doffing support, ideally outside each household. A staff member should be able to collect, put on, remove and dispose of PPE without walking through a resident lounge or leaving contaminated items in a corridor.

Manage personal belongings without making rooms institutional

Personal belongings help a room feel like home, but crowded surfaces make cleaning harder. Provide enough resident storage and agree with residents and families how items will be arranged so that high-touch areas remain accessible. This is a practical balance between homelike design and reliable environmental hygiene.

Design staff facilities as part of infection control

Staff rooms should be dedicated, away from resident areas and not accessed through resident spaces. Staff changing and hygiene facilities should be provided, with the ability to segregate them further when circumstances require. Separating staff support areas reduces avoidable traffic through households and gives staff a controlled place to change, wash and manage uniforms or PPE.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

These facilities also affect compliance. If changing, handwashing or PPE disposal requires a long detour, staff are more likely to improvise. Locate them on routes that are convenient but do not become shortcuts through resident areas. Confirm that the arrangement meets accessibility, fire, waste and occupational-health requirements.

Turn the layout into an outbreak-ready zoning plan

  1. Map the normal state. Record each household, resident group, staff assignment, entry, exit, dining area, bathroom, outdoor route, storage point and air-handling zone.
  2. Identify points of mixing. Mark shared lifts, corridors, dining rooms, therapy spaces, bathrooms, staff routes and delivery paths. Note where a resident or staff member must cross another household.
  3. Define alternative zones. Decide which rooms, household areas or wings could contain exposed, suspected and unaffected groups under clinical direction. Keep routes, toilets, waste handling and PPE points workable in each scenario.
  4. Check air movement. Review outdoor-air supply, exhaust paths, recirculation, pressure relationships and filtration with a qualified professional. Use CO₂ spot checks as an air-quality proxy, recording which reference value the facility is applying and why.
  5. Test the plan with staff. Walk the routes while carrying supplies, assisting a resident or wearing PPE. Correct blocked doors, missing storage, confusing signage and unsafe crossing points.
  6. Coordinate with operations. Align the plan with cleaning schedules, staffing, visitor controls, laundry, food service, waste removal, clinical escalation and communication procedures.
  7. Review after exercises and incidents. Update the zoning plan when the building, HVAC system, resident profile, staffing model or applicable regulation changes.

How hospital guidance should—and should not—be applied

WHO’s Natural ventilation for infection control in health care settings (2009) is principally written for engineers and architects working on healthcare facilities. It focuses on natural ventilation for airborne-precaution rooms and adjacent spaces and does not comprehensively cover every infection-prevention measure. It is useful technical context, but it is not a complete aged-care design standard.

CDC environmental-control recommendations include detailed requirements for specialised healthcare spaces such as protective-environment rooms and airborne-infection isolation rooms. Those specifications address defined room types, including filtration, sealing, pressure relationships and air-change performance. They should not be presented as blanket requirements for every aged-care bedroom. Australia’s aged-care IPC guide notes that isolation or negative-pressure rooms and airflow-pressure monitoring may not be feasible in all aged-care settings.

The Aged Care Quality and Safety Commission’s ventilation resource, published in 2022, describes healthcare-setting guidance for improving ventilation systems and reducing transmission risks. Its information may need updating to align with newer aged-care legislation and standards. Verify current local requirements before specifying equipment or pressure regimes.

Common design mistakes and their corrections

  • Relying on a single CO₂ number. The Australian guide contains both 1000 ppm and 880 ppm references. Document the chosen interpretation and treat CO₂ as a proxy, not an infection test.
  • Assuming portable filtration fixes poor ventilation. Use HEPA devices as a supplement and obtain a whole-system assessment where air quality is unacceptable.
  • Creating “zones” that share one unavoidable corridor. Rework routes, entries, dining and bathroom access so separation is practical, not merely labelled.
  • Installing isolation rooms without an operational plan. Confirm staffing, observation, PPE, cleaning, waste, clinical escalation and transfer arrangements before relying on the room.
  • Putting PPE in a distant store. Provide discreet donning and doffing support near each household and keep clean supplies protected.
  • Specifying finishes that cannot tolerate routine cleaning. Confirm chemical compatibility, repairability and access to every high-touch surface with the cleaning team.
  • Forgetting staff movement. Include staff rooms, changing and hygiene facilities in the infection-control layout, rather than treating them as back-of-house afterthoughts.

Project checklist for architects and operators

  • Fresh-air access, exhaust routes and filtration have been assessed by a competent professional.
  • CO₂ monitoring locations, action levels and the distinction between the guide’s 1000 ppm and 880 ppm references are documented.
  • Household size, dedicated staffing, boundaries and closable entries support cohorting.
  • Dining, living and activity spaces can operate in smaller groups, with at least one separable living area.
  • Single rooms with external windows and ensuites are provided where feasible; shared rooms preserve privacy and cleaning access.
  • Handwashing, PPE, clinical supplies and waste points are close to the work they support.
  • High-touch finishes are durable and compatible with approved cleaning products.
  • Staff changing, hygiene and rest facilities do not require travel through resident areas.
  • Outdoor areas are reachable without routing residents through another household.
  • The outbreak plan includes zoning options, route maps, staffing assumptions and regular exercises.
  • Local building, fire, accessibility, health and aged-care requirements have been checked before construction or HVAC changes.
