For a busy hospital or clinic route, an automatic sliding door is usually the strongest starting point because it preserves the opening without a swinging leaf and can accommodate wheelchair, bed and trolley traffic. It is not automatically the right answer for every room. The correct system depends on traffic, clear width, sensor coverage, fire and evacuation strategy, accessibility law, infection-control and ventilation needs, security, maintenance access and reliable power.

NHS England recommends automatic doors in areas of high use and says automatic sliding doors are preferable to automatic swing doors in the context covered by Health Building Note 00-04. That document’s dimensions are guidance for its stated healthcare applications, not universal building-code minima. U.S. projects must apply the 2010 ADA Standards and adopted building and fire codes; projects in Australia and New Zealand must resolve the requirements in the applicable jurisdiction and the Australasian Health Facility Guidelines.

What automatic doors solve in a healthcare facility

Automated doors allow a person to pass without pulling a leaf, reaching for a handle or stopping a bed to clear the swing path. That can improve circulation at entrances, diagnostic departments, operating-suite support areas, imaging, emergency routes and other high-use locations. The benefit is functional: fewer conflicts between the door leaf and people or equipment, easier passage for wheelchair users and less interruption to trolley movement.

Automation does not make a doorway inherently accessible, hygienic, fire-safe or suitable for a particular ward. A door can still have an inadequate clear opening, poorly positioned controls, blind sensor zones, unsafe closing force, an unsuitable fire rating or no workable failure mode. Treat the operator as one part of the door assembly, wall, controls, detection, access control, alarm and power system.

When automation is justified

  • High-volume routes where users repeatedly move through the opening.
  • Routes used by beds, stretchers, wheelchairs, carts or people carrying clinical equipment.
  • Entrances where hands-free operation supports the functional brief, while still providing a way to operate the door during a fault.
  • Locations where a swing leaf would obstruct circulation, a corridor or a bed-positioning area.

Automatic doors are not required in every healthcare room. A manual door with a closer, hold-open device or other compliant arrangement may be more appropriate where traffic is low, privacy is important, a fire compartment must remain closed, or reliable automation cannot be maintained.

Which automatic door type fits the route?

The Australasian Health Facility Guidelines identifies sliding, swing, folding, revolving and roller-shutter automatic doors. Selection should start with the route and the people and equipment using it, not with a catalogue model.

Door type Typical healthcare use Strengths Design cautions
Automatic sliding Main entrances, high-use corridors and routes serving wheelchairs, beds or trolleys No projecting swing arc; can provide a wide opening; well suited to frequent two-way traffic Requires protected side pockets and overhead/operator space; sensors must cover the whole opening; sliding leaves need a defined emergency and power-failure mode
Automatic swing Existing hinged openings, lower-volume rooms or locations where a sliding pocket cannot be built Can suit a conventional frame and may be simpler to retrofit The leaf can strike a person or equipment; the swing area needs warning and protection; egress and break-out behavior require careful coordination
Automatic folding Openings where depth is limited but more clear width is needed than a single swing leaf provides Uses less side-pocket length than a sliding system Panels and hinges add pinch and maintenance points; verify the fully folded position, clear width and sensor coverage
Automatic revolving Some public entrances where continuous flow and environmental separation are priorities Can moderate drafts and maintain separation between internal and external air Not a general solution for beds, stretchers or emergency evacuation; provide an accessible alternative and confirm fire and egress strategy
Automatic roller shutter Service openings, loading areas or security separations rather than ordinary patient circulation Provides a compact, secure closure for a large service opening Usually unsuitable as the sole everyday patient route; check emergency release, impact protection, fire performance and cleaning of the curtain

Automated systems generally cost more than non-automated alternatives such as hold-open devices and door closers, according to the Australasian guidance. That higher initial cost buys capability only if the opening, controls, power and maintenance plan are designed together.

Clear widths and accessibility: keep jurisdictions separate

NHS England guidance

Health Building Note 00-04, whose door guidance was last updated 16 April 2013, gives two important examples for automatic double sliding doors: a minimum clear width of 1200 mm for wheelchair traffic and 1740 mm for bed or trolley traffic. These are figures in that NHS guidance for the described applications, not universal legal minima. Confirm the current edition, the room schedule and the adopted standards for the project.

The same document recommends automation in areas of high use and states that sliding doors are preferable to swing doors in the described context. It also says the doors should open automatically in a power failure or when rapid evacuation is needed, referring readers to BS 7036:1996. Treat that wording as NHS guidance to be checked against the current fire strategy, product certification and local requirements.

United States ADA requirements

The U.S. Department of Justice 2010 ADA Standards specify at least a 32-inch (815 mm) clear opening for automatic doorways in both power-on and power-off mode. The requirement sits alongside rules for maneuvering clearances, thresholds, controls and other accessibility details. A nominal door size is not proof of compliance: measure the unobstructed opening in the operating states that users will encounter.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

For certain automatic doors on a means of egress without standby power, the ADA Standards also specify a 32-inch (815 mm) minimum clear break-out opening in emergency mode. Whether that condition applies depends on the door type, egress arrangement and available standby power. Coordinate accessibility, life safety and electrical design rather than treating the ADA dimension as a complete door specification.

Australasian projects

The 2025 Australasian Health Facility Guidelines provides a design framework for healthcare doors, but the adopted requirements still depend on the project location and authority. Use its door-type and operational guidance with the applicable accessibility, fire, building and healthcare standards; do not substitute NHS or U.S. dimensions for local requirements.

Sensor, control and safe-closing details

Detection must protect every legitimate user, including short users, children and guide dogs. NHS HBN 00-04 says sensors should detect all users and prevent closing while the doorway is not clear. Where floor sensors are used, the document says they should respond to uneven pressure, such as pressure from crutches. Specify the detection technology, coverage diagram and fail-safe behavior; do not accept a generic statement that a sensor is “safety rated.”

Push buttons and activation devices

For the NHS-described arrangement, push-button controls should be outside the fully open door swing, mounted between 750 and 1000 mm above the floor, visually contrasted and approximately 50 mm square. A control that is hidden behind the leaf, too close to the jamb or difficult to distinguish can make an otherwise compliant door impractical. Confirm reach ranges, approach clearances, infection-control requirements for the control surface and the access-control sequence.

Managing swing hazards

Where an automatic swing door is used, mark or otherwise warn users about the swing area and protect the swept path from beds, carts and pedestrians. Check opening speed, hold-open behavior, closing force and the interaction between presence sensors and activation devices. A detector that sees the approach but not a person standing beside the leaf is not adequate coverage.

Fire, evacuation and power-failure behavior

Every automatic door needs a defined state for normal operation, fire alarm, loss of normal power, loss of standby power and a mechanical or electronic fault. The answer may be “open,” “closed,” “manual,” “break out” or a controlled combination, depending on whether the door is in a compartment line, an egress route, a smoke-control boundary or a security zone.

England

NHS Health Technical Memorandum 05-01, Managing Healthcare Fire Safety, was updated on 11 February 2026 and should be read with the other Firecode memoranda, including HTM 05-02 and HTM 05-03. The UK Home Office fire-risk-assessment guide for healthcare premises was updated on 12 September 2023 and covers hospitals and medical centres in England, while excluding some residential and other care settings. Use those documents with the project’s fire strategy, door rating and evacuation plan.

Those are statements from NHS England HBN 00-04, sections 8.44 and 8.45. They are not a blanket instruction to automate every door or to ignore the current requirements of the authority having jurisdiction.

United States

On a U.S. egress route, verify whether the operator has standby power, whether emergency break-out is required and how the door behaves when power or detection is lost. The 2010 ADA Standards’ power-on, power-off and break-out provisions must be coordinated with the adopted building, fire and electrical codes. Document the tested clear opening in each required mode.

Australasia

The 2025 Australasian guidance says automatic doors should be serviced by generator-backed essential power for continual operation. It does not require a UPS where that backup essential supply is available; a UPS may be required if backup essential supply is unavailable or if the project requirements call for one. This is guidance for its stated jurisdictional context, not a universal power rule.

Infection control, ventilation, privacy and security

Hands-free activation can support the functional brief, but no automatic-door type should be described as preventing infection. Select finishes, seals, tracks, push plates and sensor housings that can be cleaned with the facility’s approved products. Avoid ledges and recesses that collect dust or fluid, and make sure maintenance access does not require contaminating a clean area.

Door movement changes airflow. For isolation rooms, operating areas, sterile stores and other pressure-controlled spaces, coordinate the opening sequence with the mechanical design so the door does not remain open longer than the clinical brief permits. Sliding and revolving systems may influence drafts and acoustic separation differently; verify privacy, sound transmission and observation-panel requirements with the room data sheet.

Access control must not defeat free egress. Confirm how card readers, intercoms, request-to-exit devices, fire alarms and emergency releases interact, and define who can enter when the operator is in a degraded mode.

A practical specification sequence

  1. Classify the route. Record whether it is a public entrance, clinical corridor, trolley route, service opening, compartment boundary or egress door. Identify fire and smoke ratings, security level and pressure-control needs.
  2. Count and characterize traffic. Note peak two-way use, wheelchair users, beds, stretchers, carts, children and guide dogs. Include unusual loads such as bariatric equipment or portable imaging devices where relevant to the brief.
  3. Set the required clear opening. State the measured clear width in normal, power-off and emergency modes. For NHS work, test the 1200 mm wheelchair and 1740 mm bed/trolley figures where they apply; for U.S. work, test the ADA 815 mm requirement and any break-out opening.
  4. Select the door family. Compare sliding, swing, folding, revolving and roller-shutter options against side-pocket space, swing hazards, privacy, cleaning and traffic flow. Record why a non-automated option was rejected or accepted.
  5. Draw detection and controls. Show sensor fields, floor detection, activation buttons, access-control readers, warning zones and approach clearances. Verify detection of people who stand still, use crutches, move slowly or are below the main sensor field.
  6. Define every operating mode. Write the behavior for normal operation, fire alarm, evacuation, power failure, standby-power failure, sensor fault, obstruction and manual release. Include the action required from staff.
  7. Coordinate power. Identify normal circuits, generator-backed essential circuits, batteries or UPS where required, isolation points and restart behavior. Do not assume a domestic opener can be connected to a healthcare emergency circuit.
  8. Check interfaces and finishes. Coordinate frames, thresholds, floor levels, wall protection, infection-control finishes, air balance, alarms, access control and fire-stopping before procurement.
  9. Commission with users. Test clear widths, approach and presence detection, closing forces, break-out, alarm interfaces and degraded modes with wheelchair users, beds and trolleys representative of the facility’s operation. Record results and train estates and clinical staff.

Maintenance and operational continuity

Automatic doors contain operators, sensors, controls, moving hardware and safety interfaces that require planned service. The maintenance plan should identify the responsible contractor, inspection intervals set by the manufacturer and authority, spare-parts strategy, emergency call-out route and the temporary safe state if the system is unavailable.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Inspect sensor alignment and the full detection field, including floor-level coverage.
  • Check opening and closing speed, force limitation, rebound or reversal and obstruction response.
  • Verify the measured clear opening in power-on, power-off and emergency modes.
  • Test fire-alarm, access-control, standby-power and manual-release interfaces.
  • Examine tracks, rollers, hinges, break-out hardware, seals, guards and fixings for wear or contamination.
  • Keep service records, fault reports, test results and staff training records available to the estates team.

If a door is essential to patient flow, plan a controlled manual or temporary route before taking it out of service. A technically compliant installation that cannot be serviced without closing a critical corridor is an operational risk.

Retrofitting an existing opening

An “automatic door opener kit” is not automatically suitable for a healthcare facility. Before considering a retrofit, verify the frame and leaf condition, clear opening, swing clearance, operator duty cycle, sensor coverage, fire rating, break-out requirements, access-control interfaces, emergency power and local approvals. A kit designed for a light commercial door may not withstand bed traffic or meet the required failure mode. Have the selected assembly reviewed by the qualified door, fire, accessibility and electrical design teams before purchase.

Decision examples

Scenario Likely direction Questions that decide the specification
Main outpatient entrance Automatic sliding, possibly paired with a separate accessible route Peak two-way traffic, weather separation, security, wheelchair approach, emergency release and generator supply
Ward corridor used by beds Wide automatic sliding or another system proven for the required bed route Clear width, passing space, trolley impacts, fire compartments, sensor coverage and cleaning access
Small examination room Often manual, or automatic swing only where the brief demonstrates a need Privacy, low traffic, swing hazard, infection-control cleaning and whether automation adds more maintenance than value
Service/loading opening Automatic roller shutter or industrial sliding arrangement Vehicle and equipment clearances, impact protection, fire rating, emergency release and separation from patient circulation

How to answer “What type of automatic door is best for a hospital?”

For a high-use hospital circulation route, start by evaluating an automatic sliding door because it can avoid a swing arc and support wide, two-way movement. Then prove that choice against the actual clear-width requirement, bed and trolley dimensions, detection coverage, fire strategy, egress mode, power continuity, cleaning regime, privacy and maintenance access. Choose an automatic swing, folding, revolving or roller-shutter system only when the route’s constraints make its advantages more valuable than the added hazards or limitations.

Before signing off, the owner, architect, door specialist, fire engineer, accessibility adviser, electrical engineer, infection-control representative and estates team should agree the operating modes and test evidence. The governing requirements are those adopted for the project’s location, occupancy and route classification—not a dimension copied from another country’s guidance.
