Can We End Most Homelessness? Prevention and Housing First Are the Starting Point

Most homelessness can be reduced through coordinated prevention, affordable housing, rapid re-housing, and Housing First—but no evidence sets a guaranteed percentage or deadline. Here is what the research supports and what communities must build and operate.

Most homelessness is plausibly preventable or resolvable, but no evidence proves a guaranteed route, deadline, or numerical threshold for “most.” The defensible strategy is a coordinated system: stop avoidable housing loss, expand genuinely affordable homes, rapidly rehouse people who do lose housing, and provide permanent supportive housing with voluntary services for people facing complex needs. Housing First is a central operating principle in that system, not a stand-alone building type or a substitute for enough homes and staff.

The architecture question is therefore practical. Can a community create enough accessible, stable housing and operate it with the services people choose to use? Research supports that direction. It does not show that one construction method, floor plan, or design feature independently ends homelessness.

What would “ending most homelessness” actually mean?

There is no published number defining “most” and no established date by which the United States could reach it. That matters because homelessness is measured in different ways. The U.S. Department of Housing and Urban Development’s January 2024 Point-in-Time count identified 771,480 people experiencing homelessness on one night, about 23 people per 10,000 U.S. residents. The count was 18% higher than in 2023.

A Point-in-Time count covers people in emergency shelter, safe havens, transitional housing, and unsheltered locations during a particular night. It is not a count of everyone who experiences homelessness over a year. The same HUD report found the largest single-year increase among people in families with children, up 39% from 2023 to 2024. Veteran homelessness, by contrast, had declined 55% since data collection began in 2009; HUD attributes that progress to targeted, sustained funding.

HUD’s 2024 report lists affordable-housing pressures, inflation, stagnant wages among middle- and lower-income households, systemic racism, displacement, immigration, and the end of pandemic prevention measures among likely contributors to the record count. These are the report’s assessment of contributing conditions, not proof that one factor caused every episode.

A realistic policy test is narrower than a promise to reach a particular percentage: fewer people enter homelessness, people who do enter leave quickly, and housing loss is rare after placement. Those outcomes can be measured even though a national “most” threshold has not been set.

Can homelessness be prevented?

Yes, but prevention has to begin before a household reaches a shelter or an encampment. The U.S. Interagency Council on Homelessness (USICH) published the federal government’s first homelessness-prevention framework on September 23, 2024. It calls for a community-wide, cross-system approach involving housing, health, human services, justice, education, employment, child welfare, and emergency services. The framework and its federal resources are available from USICH.

Effective prevention is usually a package rather than a single check. Depending on the household’s problem, it can combine rental or utility assistance, legal aid, mediation with a landlord, case management, housing navigation, and help replacing documents or accessing benefits. A family facing an eviction deadline needs a different response from a person leaving jail without a lease, even when both are counted as “prevention.”

What program results can and cannot show

USICH’s eviction-prevention spotlight reports that 95% of households served by Minnesota’s Family Homeless Prevention and Assistance Program remained permanently housed after intervention, while 1% became homeless within 12 months. That is a reported outcome for one program and follow-up period, not a national causal estimate. Other examples in the spotlight, including programs in Tulsa, Waterville, Massachusetts, Wisconsin, and San Diego County, illustrate local designs rather than comparable controlled trials.

Housing costs make prevention urgent. USICH reports that more than 70% of extremely low-wage households spend over half their income on rent. It also cites a study of the 20 largest Continuums of Care in which every $100 increase in median rent was associated with a 9% rise in homelessness. “Associated with” does not establish a universal causal effect, but it is a useful warning: crisis services cannot compensate for a rent market that repeatedly pushes low-income households out.

What does Housing First mean?

Housing First places people in permanent housing without requiring sobriety, treatment participation, or a demonstration of “housing readiness.” Services can be offered alongside the lease, but accepting treatment or case management is not a condition of getting housed. The model treats housing as a platform from which people can address health, disability, employment, safety, and recovery goals.

That distinction separates Housing First from programs that make a person complete treatment, maintain abstinence, or progress through temporary housing before qualifying for a permanent home. Housing First does not mean “housing only.” It means the lease is not held hostage to compliance with services, while the system still supplies the voluntary, trauma-informed help needed to sustain a tenancy.

Permanent supportive housing

Permanent supportive housing (PSH) is housing without a time limit for people with disabilities who experience homelessness. Under a Housing First approach, residents receive broad, voluntary wraparound services, such as health care coordination, behavioral-health support, help with daily living, and tenancy assistance. USICH identifies PSH as a priority for people and families with complex service needs and says the existing supply is insufficient.

A PSH program therefore needs two linked capacities: an appropriate home and an operating team. Building units without funding for supportive staff leaves a service gap; hiring staff without enough leases leaves people waiting. The model is also not a guarantee that every tenancy will be permanent. People can still lose housing, and landlords and providers need fair, clear processes for responding to arrears, conflict, safety concerns, and changing support needs.

Does Housing First work?

HUD’s autumn 2024 review of the research literature reports that it has nearly reached consensus that providing permanent housing without mandated sobriety, treatment, or service participation helps people exit homelessness and achieve long-term housing stability. In a 2020 meta-analysis of four major randomized controlled trials, Housing First produced strong evidence of greater housing stability and quicker exits from homelessness than treatment as usual.

The evidence is less settled for health-care use. HUD describes the evidence for reductions in emergency-department use, hospitalizations, or time hospitalized as moderate but inconsistent. Housing First should not be marketed as a cure for mental illness, substance-use disorder, poverty, or every cause of housing loss. Its clearest demonstrated effect is housing stability compared with usual services.

USICH’s 2023 research agenda likewise describes decades of evidence for Housing First and Critical Time Intervention in helping people exit homelessness, while emphasizing that more research is needed on how to scale housing and supportive services. Evidence is limited for several prevention ideas, including guaranteed basic income, flexible funding pools, and shelter diversion. A promising local pilot should therefore be evaluated rather than assumed to work everywhere.

How the interventions fit together

Communities need a sequence of responses rather than a single program. Eligibility, duration, service intensity, and evidence differ across the housing-crisis continuum.

Intervention Point in the crisis Typical assistance Housing condition Evidence and requirements
Universal or targeted prevention Before a household loses housing Rent or utility aid, legal help, mediation, navigation, and case management Household remains in current home or secures another one Local designs vary; USICH calls for cross-system coordination and notes that evidence is limited for some newer approaches
Diversion At shelter entry or when homelessness is imminent Short-term problem solving, mediation, family or community connection, and a small amount of flexible aid Usually a safe alternative to shelter, when one exists Eligibility and outcomes differ widely by community; do not treat a pilot result as a national effect
Rapid re-housing After a person or family loses housing Housing search, deposits, short-term rent assistance, and limited case management Assistance is time-limited; the lease is generally not conditional on treatment Requires available units, landlord participation, and a plan for income or ongoing subsidy
Permanent supportive housing For people with disabilities and complex, long-term needs Permanent lease plus voluntary, trauma-informed, wraparound services Housing is not time-limited and is not contingent on sobriety or treatment participation Strong Housing First evidence for housing stability; supply and service capacity remain below need

The categories overlap in practice. A household can receive prevention help, enter shelter after an unsuccessful intervention, and later need rapid re-housing or PSH. A coordinated entry system should match the least intensive response that is safe and sufficient, while allowing people to step up or down as circumstances change.

What role does affordable housing play?

Prevention and Housing First both depend on a supply of homes that people with very low incomes can actually afford. If every vacancy requires income far above a household’s resources, rapid re-housing cannot move people quickly and PSH providers cannot offer placements. Subsidies, preservation of existing affordable units, and new construction are therefore system infrastructure, not optional add-ons.

USICH’s federal plan calls for increasing PSH for people and families with complex service needs. It also identifies prioritizing people experiencing homelessness for affordable housing, rapid re-housing, and supportive housing as Housing First approaches. The practical question for a local housing authority or nonprofit is how many units are available now, how many are accessible to people with disabilities, and how many can be paired with operating support for as long as residents need it.

What can architecture and design contribute?

Design can make a program workable, dignified, and easier to operate, but the reviewed evidence does not establish a preferred building typology or architectural intervention that independently prevents homelessness. A building cannot replace rent assistance, a legal advocate, a behavioral-health clinician, or a permanent subsidy.

Design decisions that support implementation

  • Privacy and autonomy: A private room or apartment, a lockable door, and control over daily routines can make the permanent-home promise real. These are implementation considerations, not proven stand-alone causal fixes.
  • Accessibility: Step-free routes, adaptable bathrooms, elevators, visual and acoustic cues, and units that can accommodate mobility or sensory needs reduce barriers for residents with disabilities.
  • Location: Proximity to transit, food, health care, schools, and employment can reduce the practical burden of keeping appointments and managing daily life. Site selection must be paired with actual service and transportation funding.
  • Shared space: Common rooms, laundry, outdoor areas, and private meeting rooms can support community and service delivery. They should not force social interaction or substitute for a private home.
  • Operations: Staff workspaces, secure storage, maintenance access, clear circulation, and durable finishes affect whether a provider can deliver voluntary services safely and consistently.
  • Neighborhood fit: Good projects use ordinary planning, safety, and accessibility standards rather than creating an isolated institutional environment. Community engagement is part of implementation, not evidence that a form of architecture ends homelessness.

The best design brief starts with the service model and resident needs: who will live there, what support is voluntary, which accessibility requirements are common, how vacancies are filled, and how the building will be maintained over decades.

A practical sequence for communities

  1. Define the local problem. Combine Point-in-Time data with shelter use, eviction filings, school and child-welfare data, hospital discharge information, and racial and disability-disparity measures. State clearly which data are one-night snapshots and which describe flows over time.
  2. Build a prevention triage. Create a single, reachable pathway that can screen for eviction risk, utility shutoff, unsafe housing, domestic violence, discharge from institutions, and loss of income. Route each case to the right specialist instead of sending every household to shelter.
  3. Fund flexible, fast assistance. Small, timely payments can resolve arrears or deposits, but money works better when paired with legal aid, mediation, navigation, and benefits access. Set written rules for approvals so help does not depend on personal connections.
  4. Make shelter a bridge, not a destination. Offer low-barrier access while housing staff search for permanent options. Do not make sobriety or treatment completion a prerequisite for moving to a lease.
  5. Expand the right housing stock. Reserve or create affordable units for people exiting homelessness, including accessible units and PSH. Track vacancies, lease-up time, and turnover so a nominal unit count does not conceal unusable capacity.
  6. Pair every PSH unit with operations. Budget for voluntary services, property management, repairs, tenant protections, and clinical or peer support. Establish how residents can change service intensity without losing housing.
  7. Coordinate systems that create housing loss. Align courts, hospitals, jails, schools, child welfare, employment programs, and housing agencies around discharge planning and data-sharing rules that protect privacy.
  8. Measure outcomes and adjust. Publish entries into homelessness, returns after placement, length of time homeless, housing retention, racial and disability equity, tenant satisfaction, and service reach. Separate program-specific outcomes from population-wide changes.

Common failure modes and how to correct them

  • Declaring victory from a single count: A lower Point-in-Time number can reflect weather, counting changes, or people temporarily staying elsewhere. Pair it with annual inflow, exits, and returns.
  • Building units without services: PSH cannot operate on capital funding alone. Secure multi-year service and property-management resources before opening.
  • Making treatment a lease condition: This excludes people Housing First is designed to serve. Offer engagement and harm-reduction options voluntarily, with clear safety procedures.
  • Using a local percentage as a universal promise: Minnesota’s reported retention result is informative but not comparable to every community. Report its denominator, follow-up period, and program design.
  • Confusing affordability with architecture: Attractive design does not make rent affordable. Identify the subsidy, income source, and long-term operating plan for every unit.
  • Ignoring landlord and tenant realities: Recruit landlords, pay claims promptly, provide mediation, and give tenants understandable lease and grievance information. Trust is an operating requirement.
  • Counting placements but not stability: A move-in is an intermediate result. Track whether housing is retained, whether people return to homelessness, and whether residents can obtain needed services without coercion.

How to judge whether progress is real

A credible community dashboard should show both prevention and resolution. Useful measures include the number of households receiving help before eviction, shelter entries, unsheltered episodes, median time from referral to lease, housing retention at 6 and 12 months, returns to homelessness, PSH vacancies, and the share of residents offered—not compelled to accept—services. Break results out by family status, veteran status, race, disability, age, and gender so an overall improvement does not hide unequal access.

The evidence supports an ambitious conclusion: coordinated prevention plus Housing First can substantially reduce homelessness, and targeted programs have demonstrated meaningful housing stability. It does not support promising that a particular design, program, or government can eliminate a defined percentage on a fixed schedule. “Most” becomes a responsible goal only when communities keep people from losing housing, create enough affordable and supportive homes, and fund the services that make those homes usable over time.
