How to Design a Dementia-Friendly Bathroom: Practical Criteria for Safe, Legible Spaces

Design a dementia-friendly bathroom around the person’s abilities: make fixtures legible, light evenly, provide safe shower access and support, simplify controls, reduce glare and preserve dignity.

A dementia-friendly bathroom is not defined by one colour, tap or product. It is a bathroom whose route, fixtures, controls and bathing equipment are easy for the particular person to recognise and use, while lighting, surfaces and access arrangements reduce avoidable risk and preserve privacy.

The criteria below combine current Australian and UK guidance with the original 2015 Architecture & Design framing. Use them as a design and review framework, then confirm local building, accessibility, wet-area and electrical requirements with qualified professionals.

Start with the person, the setting and the route

Dementia affects people differently, and abilities can change. A bathroom for a person living alone may need different sightlines and emergency arrangements from one in a care facility. Before choosing finishes or fittings, observe how the person approaches, enters, turns, transfers, washes and leaves the room. Ask what feels familiar, which controls are easy to operate, and whether mirrors, patterns or reflections cause distress.

Map the complete journey: bedroom or living area to bathroom door, threshold, toilet, basin, shower and exit. Note where the person pauses, loses visual orientation, needs assistance or encounters a door that is difficult to open. Include the person in decisions wherever possible; a technically accessible room that feels institutional or removes control can undermine dignity and use.

A practical assessment sequence

  1. Record abilities and habits. Note balance, walking aid or wheelchair use, transfer method, reach, grip, vision, hearing, continence routines and preferred bathing position.
  2. Identify assistance. Establish whether one or more carers need to stand beside the person, help with transfers, or reach a control without blocking the exit.
  3. Check the existing room. Look for glare, shadows, slippery or shiny floors, confusing patterns, poorly placed rails, inaccessible switches and a toilet that is hard to see.
  4. Set priorities. Deal first with falls, inability to locate the toilet, unsafe water temperature, blocked transfers and doors that cannot be opened in an emergency.
  5. Test at the person’s eye level. View the room while standing, seated and using any mobility aid. Contrast and sightlines that work for a designer may fail for the user.

Make the visual hierarchy unmistakable

Recognition depends on contrast against the actual surroundings, not on a colour name in isolation. The toilet seat, grab rails, basin, controls, door hardware and other important fixtures should be distinguishable from the wall, floor, bowl, bench or door behind them. Victoria’s Department of Health specifically advises contrasting the toilet seat with the bowl, floor and walls, and contrasting rails with the wall (Department of Health Victoria, updated 28 October 2021).

A white seat on a white toilet against a pale wall can make it difficult to identify where to sit or aim. A contrasting seat can improve recognition, but the best contrast depends on lighting, background and the person’s vision. Confirm the choice in the installed room rather than relying on a catalogue photograph. The 2015 Architecture & Design article also highlighted coloured toilet seats and contrast between a basin and its bench as core design features (Architecture & Design, 6 July 2015).

Specify contrast by element

Element What should stand out What to check on site
Toilet Seat against bowl, floor and wall It remains visible from the approach route and under night lighting.
Grab rails Rail against the wall Contrast is still clear when the rail is wet or partly shadowed.
Basin and bench Basin rim and controls against the counter Grouped fittings do not merge into one confusing shape.
Door and hardware Door leaf, handle and lock against adjacent surfaces The exit is recognisable without relying on a small sign.
Shower controls Handset, hose and controls against wall finishes The person can identify what to touch before water flows.

Keep the visual field simple. Busy motifs, abrupt floor changes and several similar-looking objects can compete with the fixtures that matter. Familiar forms may be easier to interpret than unusual designer fittings, provided they can be used safely by the individual.

Light evenly and control glare

Lighting should reveal the route, fixtures and faces without creating glare or deep shadows. Use an even layer of ambient light, supplement it where the person needs to transfer or operate a control, and check reflections from tiles, chrome, mirrors and wet surfaces. A bright fitting that produces a dark shadow under the basin can be less useful than a lower-glare, more evenly distributed installation.

Dementia Australia’s professional guidance gives an example target of at least 300 lux for even lighting in a dementia-enabling care environment. This is guidance for that context, not a universal statutory bathroom lighting level (Dementia Australia, last updated 24 September 2026). The electrician and local requirements determine the compliant installation.

Night-time cueing

A passive-infrared light that switches on when movement is detected can help a person locate the toilet at night without searching for a switch. Automatic lighting is an option, not a substitute for a safe route: test its brightness, activation range, shadows and effect on sleep. The NHS also identifies automatic light sensors as a useful addition (NHS, reviewed 23 January 2025).

Place the light so it illuminates the floor and toilet without shining directly into the person’s eyes. Provide a manual override where appropriate, and make sure a power failure does not leave the route dangerously dark.

Design the shower for access, assistance and transfers

A step-free shower is preferable where the building and drainage allow it. It removes a threshold that can catch a foot or mobility aid and creates a clearer route for a carer. The shower zone must be large enough for the person, any walking aid or wheelchair, and the number of assistants required. Do not reduce manoeuvring space to gain a larger vanity or storage unit.

  • Use a non-slip shower floor; wet-area slip resistance still needs to be selected and installed to local requirements.
  • Provide a flexible handheld shower that can be used seated or standing. The handset should be easy to grip, and the hose should be secured so a dropped handset will not strike the floor.
  • Choose a shower seat with stability and support suited to the person. Victoria recommends strong back and arm support, a secure locking mechanism and, ideally, adjustable height (Department of Health Victoria).
  • Position controls where the person or carer can reach them without leaning across a hot-water stream or an unstable transfer.
  • Keep the shower entrance and exit clear of bins, hampers and loose equipment.

Standing and seated bathing are not competing universal solutions. Balance, endurance, transfer ability, available assistance and personal preference determine which arrangement is safer and more acceptable. Reassess if the person’s mobility changes.

Use grab rails as engineered support, not decoration

Provide rails beside the toilet and in the shower where the person needs support. Rails must be securely fixed to suitable wall construction; adhesive bathroom accessories and towel bars are not substitutes. Position and height should be selected with the person, an access professional or occupational therapist, and a qualified installer, taking account of the transfer direction, reach and mobility aid.

Fixed rails offer a predictable handhold. Fold-down rails can provide clearance or serve users who approach from different directions, but they need a reliable locking action and enough space to operate. Victoria describes fixed and fold-down options and recommends height adjustment where possible (Department of Health Victoria).

If a towel rail is deliberately designed and installed to double as a grab rail, the Victorian guidance specifies resistance to a downward force of 110 kg. That is a recommendation in that guidance for this particular use, not a universal product rating or building-code requirement. Confirm the applicable local standard and the installed wall’s capacity.

Choose controls the person can recognise and operate

Controls should be visible, reachable, easy to grip and consistent with the person’s habits. Mark hot and cold clearly; Victoria recommends distinct red and blue markings. A person who has always used separate hot and cold taps with a central spout may understand that arrangement more readily than a single lever or mixer. Conversely, another person may find a lever simpler because of limited finger movement. NHS guidance likewise notes that traditional-style fittings may be preferred by some people (NHS).

There is no universally correct tap type. Check temperature-limiting provisions, the amount of force needed, the direction of movement and whether the person can tell when water is on. Avoid small basins with several closely grouped controls if they are difficult to distinguish; the original Architecture & Design summary specifically cautioned against that arrangement.

Make the toilet easy to find while protecting privacy

People may be more likely to use a toilet they can see and recognise. Dementia Australia recommends a clear sign using both the word “toilet” and a symbol, with contrasting words and pictures. Keep commonly used objects visible and avoid hiding the toilet behind visual clutter. Where appropriate, consider whether the toilet can be seen from the bed or the main sitting area (Dementia Australia).

Visibility must be balanced with dignity. A partially open sightline, a screened approach or a door with a large, legible handle may provide orientation without exposing the person. The door should be capable of being unlocked from outside in an emergency, while still giving the occupant control from inside. Agree the arrangement with the person and the care team, and check local privacy and safeguarding requirements.

Remove visual confusion from floors, walls and mirrors

Use matte, non-reflective finishes where possible. The NHS warns: “Avoid shiny or reflective flooring, as this may be perceived as being wet, and the person with dementia may struggle to walk over it” (NHS). A wet-looking reflection can stop someone at the threshold; a busy pattern can hide the edge of a mat or the location of the toilet.

  • Avoid loose rugs and mats. NHS England Digital notes that they may be misinterpreted or create a trip concern (NHS England Digital, edited 4 August 2026).
  • Limit strong stripes, high-contrast motifs and abrupt changes that resemble steps or holes.
  • Check grout lines, thresholds and drains for confusing shadows.
  • Assess mirrors with the person. Reflections can be reassuring for some and distressing or confusing for others; Dementia Australia suggests removing or covering a mirror when it causes that response.
  • Keep cleaning products, spare equipment and waste bins out of the main visual field and away from transfer paths.

Compare options against the person’s actual needs

Decision Option A Option B Deciding questions
Rails Fixed rail Fold-down rail Which gives the required support, clearance and reliable locking action?
Bathing position Standing shower Seated shower Can the person stand safely, or is a supported transfer and seat required?
Lighting control Manual switch Motion activation Can the switch be found at night, and will automatic light cause glare or disturb sleep?
Tap control Separate hot/cold taps Lever or mixer Which matches familiarity, grip, dexterity and temperature-safety needs?
Toilet privacy More open sightline More screening How can orientation and emergency access coexist with the person’s dignity?
Colour scheme High fixture contrast Lower visual contrast Which combination is legible without creating glare, pattern confusion or distress?

Review the finished room before handover

  1. Walk the route in normal and low light. Confirm the toilet, door, rails and shower controls can be located without verbal prompting.
  2. Use the actual mobility aid, shower seat and transfer method. Check that a carer can assist without blocking the exit or pulling on a non-structural fitting.
  3. Operate every control with wet hands. Check grip, force, hot/cold identification, hose security and temperature protection.
  4. Inspect floors when dry and wet. Remove loose mats, glare sources, confusing reflections and objects that narrow the route.
  5. Test the night sensor, manual override, outside emergency release and any alarm or call system.
  6. Ask the person what feels familiar, confusing or undignified, then adjust the room rather than assuming the specification is finished.

Coordinate compliance and future change

The guidance used here spans Australian and UK homes and care environments. It is not one binding international standard, and a recommendation from one jurisdiction should not be copied as a universal dimension or code requirement. Confirm accessibility, structural fixing, waterproofing, electrical, fire and privacy obligations with the relevant local authorities and licensed trades.

Document the reasons for each choice, including the person’s preferences and assistance needs. A review date is useful because dementia, balance, vision and continence needs can change. Social Care Wales also provides dementia-friendly design guidance for care and support settings (Social Care Wales, updated 6 February 2025).

The most reliable bathroom is therefore not the one with the most specialist products. It is the one in which the person can recognise the toilet and controls, move safely, receive the right assistance and retain as much choice and privacy as possible.
