Medicine packaging design is part of medication safety: the container, closure, carton and printed information all affect how a medicine is identified, dispensed, opened, stored and used. A sound design makes critical information clear while fitting the specific product, intended users and market. There is no single package that is best for every medicine.

What medicine packaging design includes
Packaging is more than the artwork on a box. Depending on the product and market, it can include the immediate container and closure, an outer carton, printed labeling and an approved package leaflet or other product information. Each part has a different job: contain and protect the medicine, communicate its identity and instructions, and support safe handling.

The design must work as a system. A label that is easy to read cannot compensate for a package that is easy to confuse with another product; a secure closure is not useful if intended users cannot operate it safely. The relevant balance depends on dosage form, container type, intended users and applicable product information.

How packaging design supports medication safety
Make the product and critical information distinguishable
During selection, dispensing and use, users and healthcare professionals need to identify the medicine and find its important information. FDA’s May 2022 final guidance, Safety Considerations for Container Labels and Carton Labeling Design to Minimize Medication Errors, addresses safety aspects of container labels and carton labeling for human prescription drugs and biological products. It aims to promote safe dispensing, administration and use and to help minimize medication errors.
FDA’s April 2016 product-design guidance takes a broader systems approach, considering medication-error risks associated with drug and biologic product design and container-closure design. These are agency recommendations; a particular layout should not be described as legally mandatory unless an applicable regulation or approved product-specific labeling establishes that requirement.

Give information a clear hierarchy
Start by identifying the statements that apply to the specific product and the locations where they must appear. FDA’s resource hub points to U.S. provisions concerning matters such as net quantity, dosage statements, ingredients, expiration dates, lot numbers, business identity, National Drug Code, prominence and barcode labeling. The hub is a navigation aid, not a substitute for checking the governing regulation and its applicability to the product.

How U.S. and EU requirements differ
Regulatory requirements are jurisdiction- and product-specific. FDA guidance for U.S. products should not be treated as an EU rule, and an EMA process description should not automatically be applied to every national authorisation.
| Market and source | What it addresses | Scope to keep in view |
|---|---|---|
| United States — FDA, May 2022 container-label and carton-labeling guidance | Safety considerations for container labels and carton design, with the aim of reducing medication errors and supporting safe dispensing, administration and use. | Human prescription drugs and biological products; the guidance is directed to sponsors, application holders and applicants. |
| United States — FDA, April 2016 product-design guidance | A systems view of medication-error risks associated with drug and biologic product design and container-closure design. | Recommendations should not be presented as universal legal requirements absent an applicable rule or approved product-specific labeling. |
| European Union — EMA product-information materials | Implementation of agreed product-information texts in printed packaging; EMA also points to guidance on Braille and package leaflets for blind and partially sighted people. | Some pre-authorisation process details concern medicines authorised through the centralised procedure and should not be assumed to apply identically to national authorisations. |
| European Union — Commission Delegated Regulation (EU) 2016/161 | Detailed rules for safety features appearing on packaging of human medicinal products. | Apply the regulation and relevant market-specific guidance; do not reduce its framework to a generic claim about “anti-counterfeit packaging.” |
When child-resistant packaging is relevant
Child resistance and usability need to be considered together. FDA’s August 2019 guidance discusses information to support child-resistant packaging statements for covered human drug labeling, including prescription and specified over-the-counter products. FDA characterizes the guidance as its current thinking and nonbinding recommendations, except where particular legal requirements apply.

When tamper-evident packaging is relevant
For specified U.S. over-the-counter products, FDA’s May 1992 compliance policy guide addresses tamper-resistant packaging intended to give consumers visible evidence that tampering has occurred. It describes examples including film wrappers, blister or strip packs, pouches, heat-shrink bands and inner seals.

A listed technology does not by itself establish compliance: the feature must be properly designed and applied, and the labeling must identify it where required. The guide has product exceptions and historical context, so check the current requirements that apply to the particular product before relying on it for a compliance decision.
Accessibility and approved product information
For centrally authorised medicines, EMA’s pre-authorisation guidance says marketing-authorisation holders are responsible for implementing agreed product-information texts in printed packaging and describes advance submission of packaging specimens. EMA’s product-information resources also point to EU provisions on Braille and package leaflets for blind and partially sighted people, as well as standard content and format.
These details make accessibility a design and implementation concern, not an optional finishing touch. Check the relevant market’s requirements and ensure the printed materials match the agreed product information. Do not assume that a process described for centralised authorisations applies unchanged to national authorisations.

A practical way to assess a package concept
Evaluate the actual package against the product, users and intended market rather than choosing by appearance alone. Use these questions to structure a review:
- Error prevention: Can the intended user and healthcare professional distinguish the product and find its critical information during selection, dispensing and use?
- Information hierarchy: Are the applicable statements clear on the container and carton, and have the relevant governing provisions been identified?
- Child resistance: Does a child-resistant feature or claim apply, and is any associated statement properly supported?
- Tamper evidence: If the product is subject to a relevant rule, does opening leave visible evidence, and does the labeling identify the feature where required?
- Accessibility: What Braille or package-leaflet arrangements apply in the intended market, and are approved texts implemented correctly?
- Regulatory fit: Does the design suit the dosage form, container type, approved product information and intended market?
Some goals can conflict. For example, closure security and accessibility may need to be considered jointly. The cited FDA and EMA materials do not establish a universal scoring formula or a single best package; the applicable requirements and the needs of the actual product and users determine the appropriate design.
