Use this simple reward card to help a child notice and practice one clear, positive behavior. Print it or copy it onto paper; the important parts are an understandable goal, a way to mark progress, and a reward the child knows how to earn.
Printable child reward card template
Copy this table into a document or draw it on paper. For a young child, use a small picture or sticker in the progress boxes; an older child may prefer checks or points.

Choose a chart format that fits the child
There is no established evidence in these guidance sources that an elaborate or purchased template works better than a homemade page. Choose the simplest format your child can understand and that you can use consistently.

The American Academy of Pediatrics (AAP) discusses tokens for early school-age children and points or contract systems for older children in Positive Reinforcement Through Rewards, last updated November 6, 2023. For young children, the CDC recommends pictures and simple words in its Tips for Using Praise and Rewards guidance.
Set a goal and reward the child can understand
- Keep the goal small and specific. Write an action the child can do, such as “use polite words,” “keep hands and feet to myself,” “brush my teeth,” or “take my plate to the sink.” Avoid vague goals such as “be good.”
- Agree on what earns a mark. Explain the goal and show the child how progress will be recorded before starting. If the chart covers several days, state exactly how many marks or successful days earn the reward.
- Choose a meaningful reward together when practical. It can be an activity or another non-material reward; buying something is not required.
- Make the time frame manageable. Young children may benefit from seeing progress marked promptly. A daily or weekly chart can suit goals that take longer to practice. The AAP advises being clear about the points or days needed and suggests a week or longer depending on the behavior.
Use the card to recognize success
- Place it where it is useful. Keep the card somewhere visible and convenient for the child and adult.
- Mark progress promptly. When the child does the agreed action, add the sticker, check, stamp, or point so the recognition is connected to the behavior.
- Praise the exact action. Say what the child did well rather than offering only a general “good job.”
- Give the agreed reward when the threshold is reached. Do not change the rules after the child has earned it.
- Adjust the plan as the behavior changes. If the chart is not helping, reconsider whether the goal, mark threshold, time frame, or reward is a good fit. As the behavior becomes established, choose a new target or phase out the chart.
Use the chart to record successes, not to take away marks or earned rewards. The AAP advises avoiding penalties and demerits; the CDC says, “Never take away rewards your child has earned.”

Make a simple card without printing
A piece of paper, a marker, and a few drawn boxes are enough. Invite the child to help choose the goal, draw the progress area, or decorate the card. Stickers are optional: a drawn symbol, stamp, or check can work as the mark.
